The current incarnation of my evaluation form has only one page (in addition to the CWU SEOIs). It provides you with an opportunity to evaluate the learning and teaching tools that we used during this quarter. If you wish to receive credit in the course for providing this valuable input, you must enter your name in the space at the top of the input form.
